Admission/Discharge/Transfer (ADT)

Admission/Discharge/Transfer Module (ADT):

This module helps in tracking the resident data like the Admission information, Discharge information, Bed retention, Leave, and Transfer information. This data is necessary for the Accounts Receivable, General Ledger, MDS, Care Plans, Pharmacy and Dietary modules for various clinical and financial processes.

This module captures Pre-admission and Admissions details.

Pre-admissions:

Pre-admissions section captures prospective Residents, personal, Demographic, billing & Clinical information. This unique pre-admission process utilizes a straight forward question-answer approach.

Admissions:

The residents details captured in the pre-admissions are moved to admissions for admitting the resident to facility. This establishes resident admission using pre-admit or prior-admission data.

Key feature:

The admitted residents will be allotted a UHID (Unique Health Identification Number) which is unique for each resident. The same number can be used for accessing the medical record or clinical information of the resident irrespective of the number of times he/she is admitted to the facility.

UHID:

Unique Health Identification Number is Auto generated. The format is First 2 characters State code, next 2 characters facility code, next 2 characters unique code and then original pay source.

Salient features:

The clinical assessment helps in capturing all the clinical information of the resident during the time of admission. This information captured will be reflected in the MDS module and is useful for the clinical staff for creating the MDS assessments and designing of care plans. All the contract forms & reports can be generated through this module.

Census reports include:

Face Sheet, Admission Form, Census Listing, ADT Report, and Graphic Census Report.

Using a Centralized Database saves time, and reduces errors by eliminating redundant data entry that normally introduces new errors. With a Central Database, information is entered once and becomes available to all applications.
