Virtual Patient Care
Remote System of monitoring SNF (Skilled Nursing Facility) patients, the “Virtual Patient Care” adds an extra layer of monitoring for the SNF sick patients using software (GeroPro- built in-house) and other electronic technology. It does not necessarily change anything at the patient’s bed side but it does provide an extra set of eyes to look at things and watch trends and catch things quicker than they would at the SNF’s.
Processes at nursing facilities are replicated at Promantra. Virtually information is gathered from the nursing stations and fed into the GeroPro modules. This process is automated by the use of PDA (pocket PC) by nurses and Interdisciplinary Teams.
- On verifying the vital data received at Promantra, Clinical process team cross verifies, validates and identifies the key areas of risks, updates care plans which are implemented by the facilities.
- Root cause analysis/Corrective measures are developed and implemented on a case to case basis
- Primary aim of CPA is to give Quality Patient Care
- Complies with Quality Audits, Survey Standards and Guidelines.
